Primary signet ring cell carcinoma of the colon: A rare condition with a poor prognosis. A report on two cases.

چکیده

El carcinoma colorrectal con células en anillo de sello es un subtipo infrecuente del adenocarcinoma mucinoso, constituyendo menos del 1% del total de tumores colorrectales1. Para el diagnóstico, debe existir un origen primario en colon o recto y al menos un 50% con patrón de células en anillo de sello. Habitualmente la presentación clínica es tardía, asociando una alta agresividad2. Describimos 2 casos de pacientes con carcinoma de colon con células en anillo de sello. El primer caso fue el de una mujer de 89 años sin antecedentes familiares de cáncer de colon con cuadro clínico insidioso de 3 meses de evolución de anorexia, pérdida de peso y dolor abdominal. Analítica con anemia hipocrómica, leucocitosis sin neutrofilia y proteína C reactiva elevada. La tomografía computarizada (TC) objetiva un engrosamiento irregular en ciego, con infiltración de grasa pericecal y pared de íleon terminal sin signos de obstrucción; con múltiples adenopatías regionales y retroperitoneales (fig. 1A). Se realiza colonoscopia larga que detecta una masa ulcerada y estenosante que ocupa la práctica totalidad de la luz del ciego e impide la progresión del endoscopio, con biopsias positivas para adenocarcinoma con células en anillo de sello. Se realiza hemicolectomía derecha oncológica, apreciándose infiltración a nivel de peritoneo parietocólico derecho y mesocolon. Examen anatomopatológico: adenocarcinoma de colon pobremente diferenciado con > 50% de patrón de células en anillo de sello (fig. 1B) con múltiples nódulos tumorales e innumerables embolismos linfáticos.
